Pulmonary responsiveness to bronchoconstricting agents is a marker of neutrophil-mediated inflammation in asthma. In asthma with chronic inflammatory response to allergens, phytohemagglutinin (PHA) bronchial challenge is used to characterize the specific immune processes involved. However, the dose of PHA to be used in such studies is not clearly defined. We performed pulmonary responses to bronchoconstrictor agents and PHA in asthmatic subjects with varying severity of airflow obstruction. We studied 45 asthmatic subjects (33 with mild, four with moderate, and eight with severe asthma) who underwent methacholine bronchial challenge and two PHA challenges. To standardize the dose of PHA, we performed bronchial challenge testing with 2.5 microg of PHA, 3.125 microg of PHA, and 5.0 microg of PHA in asthmatic subjects with mild or moderate airflow obstruction. PHA challenge testing was discontinued in four patients because the induced sputum eosinophil counts increased. PHA challenge induced sputum eosinophilia (greater than or equal to 300 cells/microL) in all seven asthmatic subjects with moderate airflow obstruction who underwent PHA challenges.
