Absorption of drugs by the digestive tract is a major determinant of drug efficacy. The P-glycoprotein (P-gp) encoded by the MDR1 gene is a transmembrane protein and an efflux pump for a wide variety of xenobiotics including both endogenous and exogenous chemicals. The expression and function of P-gp in intestinal epithelial cells is well characterized. The aim of the present study was to elucidate the possible relation between the level of P-gp protein expression and disease activity in duodenal mucosa of patients with Crohn’s disease (CD). The expression of P-gp protein was immunohistochemically evaluated in the duodenal tissues of 30 patients with CD. As compared to the control group, the P-gp level in the epithelial cells of the duodenum of patients with CD was increased markedly. There was a positive correlation between P-gp expression and the disease activity as assessed by Crohn’s Disease Activity Index (CDAI) score (r = 0.46, P = 0.01). P-gp may represent one of the determinants of drug absorption in the upper small intestine. The increased expression of P-gp in the duodenal mucosa of patients with CD may affect the efficacy of orally administered drugs.Q:
