this study is to examine whether a combination of preoperative spectral domain optical coherence tomography (sd-oct) and preoperative ocular coherence tomography angiography (octa) is useful for predicting the development of ftmh after vitrectomy. ten consecutive eyes of 10 patients with ftmh after primary vitrectomy were enrolled in this study. we performed sd-oct using the cirrad 9000 with an enhanced depth imaging (edi) protocol. sd-oct images were evaluated qualitatively for the presence or absence of the foveal crack sign. we also performed octa using the cirrad 9000 with a high resolution (hr) protocol. we qualitatively evaluated the capillary network density in the superficial capillary plexus in the central fovea using octa. we performed a multivariate regression analysis to assess the independent predictive factors associated with the foveal crack sign on sd-oct images. foveal crack sign was identified in seven of 10 eyes with ftmh by using sd-oct. in these 7 eyes, the foveal crack sign and capillary network density in the superficial capillary plexus were identified in all 7 eyes with ftmh after vitrectomy. the foveal crack sign was associated with the best-fit line intercept as a significant predictive factor (p = 0.0037). in addition, the foveal crack sign was associated with preoperative capillary network density (p = 0.0014). foveal crack sign detected on sd-oct images is associated with the best-fit line intercept and preoperative capillary network density in the superficial capillary plexus. therefore, we consider preoperative sd-oct and octa to be useful for predicting the development of ftmh after vitrectomy. this approach is expected to be more valuable in daily practice, since it can be performed with noninvasive, noncontact and non-demanding procedures.
